Airline Mileage Credit Request Form

Use the form below to request credit to your account. Retroactive credit will be given for flights taken no more than 30 days prior to enrollment into the SkyMiles® program. You must submit your mileage credit request within nine months of the date of your flight or a claimed discrepancy cannot be considered for credit.

Please enter the first 13 digits of your ticket number and your SkyMiles account number. Your 13-digit ticket number can be found on your E-Ticket Trip Summary Receipt or ticket (If your ticket number is 14 digits, please omit the last digit). If Delta is unable to locate your ticket information, you will be prompted to complete the online mileage request form.

All requests for mileage credit are subject to verification and approval by Delta Air Lines.
